Year 7 secondary school vaccine program - information and consent form
Information on Year 7 secondary school immunisation program and consent form.
Adolescents in Year 7 of secondary school (or aged 12 to 13) are recommended to receive the human papillomavirus (HPV) and diphtheria, tetanus, and pertussis (whooping cough) vaccines.
Parents, guardians, or other medical treatment decision makers must provide consent for each vaccine for adolescents to receive vaccinations at school.
The first section provides information on HPV, diphtheria, tetanus, and pertussis (whooping cough) vaccines, and then the consent form is attached at the end.
