This brochure uses reader-friendly language to inform and empower patients/consumers, helping them get the most out of conversations with their doctor, nurse or other health professional. It encourages people to speak up—ask questions, share concerns, and raise anything that doesn’t feel right or isn’t understood.

It explains that a good conversation is a two-way dialogue where the patient’s goals, values and concerns are included in the agreed care plan.
